Living Well with Diabetes

Certified Diabetic Educators (CDEs) are health care professionals who have advanced education and experience with people who have diabetes.

What Would a CDE Do for Me?

A Certified Diabetic Educator (CDE) will focus on helping you to live well with diabetes.

Diabetes self-management education includes:

  • Meal planning
  • Physical activity guidelines
  • Blood glucose and blood pressure monitoring
  • Prevention of complications
  • Medications
  • Problem solving
  • Community resources and support groups

We also offer:

  • Continuous glucose monitoring
  • Insulin pump therapy

What Will it Cost?

Charges will vary depending on the length of service and are usually covered by Medicare and private insurance companies. Outpatient Diabetes Services will verify eligibility and benefits with private insurers.

How Can I Contact a CDE?

Consultations for Diabetes Self Management and Medical Nutrition Therapy require a physician referral. Consultation is available with a certified diabetic educator (CDE) on an individual basis by appointment.

Diabetic Support Group

If you have diabetes, or are interested in learning more about diabetes, you are welcome to attend a free support group with Teresa Hill, Clinical Dietician, CDE, the second Monday of each month, September through May, from 10 to 11 a.m. 920-887-6631.

 

Prediabetes Class

MMC-BD offers educational classes for those with Prediabetes. In this one hour session, a MMC-BD Diabetes Educator will review basic information about prediabetes as well as practical tips to healthy eating, weight control and exercise. 

 Outpatient education counseling can help lower their risk of developing diabetes. To register or receive more information, call 920-887-6631.

 

Blood Glucose Meter Accuracy Clinic

All blood-glucose meters should be checked for accuracy annually by comparing results to a lab result. Minimum of 4-hour fast required. Appointments required by calling Laurie Schroeder at 920-887-6631. Fee is $2.
