Hillside Community Support Services is a program that serves the elderly and those with chronic health problems in the Dodge County and surrounding area.
The goal is to use a “whole person” approach, resulting in improved health outcomes and quality of life for those served.
Hillside Community Support Services helps individuals to continue living in their homes and can provide peace of mind for relatives unable to provide on-site supervision and support of a family member. The scheduled visits of an experienced staff person in the home, who not only performs tasks; but can observe changes in the client’s status, add to the safety network.
Individual payment is made on a private pay basis. Funds for specific services may be available from county agencies when financial and personal eligibility requirements are met. Family Care is also available for those who qualify. The number of services or visits is coordinated with each person/family to meet individual needs.
