Beaver Dam Community Hospital has a limited Swing Bed Program is ideal for persons who could benefit from short-term, skilled care when discharged from acute care. For individuals who would benefit from an additional week of rehabilitation following an acute injury or surgery, to those requiring a skilled level of care for a limited time, participation in the swing bed program allows for a smooth transition from hospital to home.

Beaver Dam Community Hospital’s Social Service professionals are available to you and your family for assistance in planning for your discharge home. Home delivered meals, medical equipment, and home care services are just a few of the services available to you, if needed, when you are discharged from the hospital.
