Certified Diabetes Educators

Who is a C.D.E.

Certified Diabetes Educators (C.D.E.) are health professionals who have advanced education and experience in teachin information and skills for living with diabetes.

What would a C.D.E. do for me?
A Certified Diabetes Educator (C.D.E.) will help you learn to live well with diabetes.

Diabetes self-management education includes:
            Meal planning
            Physical activity guidelines
            Blood glucose and blood pressure monitoring
            Prevention of complications
            Medications
            Problem solving
            Sick Day Plan
            Community resources and support groups

We also offer:
            Continuous glucose monitoring
            Insulin pump therapy

What is the cost?
Charges will vary depending on the length of services and are often paid for by Medicare and private insurance companies.

How do I contact a C.D.E.?
Ask your doctor to refer you or you may call for individual outpatient diabetes counseling. Counseling is available with a certified diabetes educator (C.D.E.) on an individual basis by appointment. Contact:
