Hillside Manor

Hillside Manor is a Medicare/Medicaid certified skilled care nursing home. The state-of-the-art facility, adjacent to Beaver Dam Community Hospital, offers skilled nursing, comprehensive rehabilitation services, as well as an extensive activities program. Activities are designed to meet the needs of each resident, with both individual and group programming.

At Hillside Manor, each resident is acknowledged as an individual. We believe that true quality care is driven by quality of life and is centered on the individual needs and choices of each resident. Home is where the HEART is, and at Hillside Manor that means:

		Home: We believe in creating an environment that we ourselves would want to call home - a comfortable home filled with family, friends, spontaneity, love, and laughter. To achieve this "family-at-home" atmosphere, residents on each floor will live in four areas, called "homes." Daily living and activities will be done primarily with that same smaller group of people. Meals will be buffet-style to give more choice of menu and will be served over a longer time to allow flexibility in schedules. Bath time will include more "spa-like" pampering. Activities will be more spontaneous, allowing "resident-families" to choose "what they want to do and when."
		Empowerment: We believe in honoring individual choices and in the right to exercise control over decisions and daily routines. Residents will be able to choose when to go to bed and when to get up in the morning, when they want to have their bath, what snacks they prefer, and which activities they want to do.
		Appreciation: We embrace each person's individuality and appreciate his/her unique gifts. We believe in the importance of acknowledging each person's accomplishments with personal expressions of congratulations and gratitude. Birthdays and other special times will be celebrated in the smaller "home" group to enhance the family-like feeling. To meet the residents' needs for purposeful, meaningful activity, they will have opportunities to contribute with home-like tasks such as setting the table if desired.
		Relationship: We believe that the quality of each relationship is important and treat residents and coworkers with kindness and respect. Each "home" will have the same staff each day; thus, the "home" will have continuity of residents, nurses, nurse aides, housekeepers, and social workers, making possible stronger and deeper relationships, again, like a family at home.
		Teamwork: We believe that through working together we will achieve a consistently high standard of quality of care and quality of life. Staff will work as a team to provide whole-person care, meeting the physical, mental, spiritual, and social needs of each resident.
