.... is a program designed to serve the elderly and those with chronic health problems in the Dodge County area. In-home services provided include: case management, direct care by a Registered Nurse, social work visits, as well as bath aide and home-maker visits.

The goal of Hillside Community Support Services is to use a holistic "whole person" approach resulting in improved health outcomes and quality of life for those served.

Services Available
Registered Nurse or social work case management visits for:
      ~ comprehensive, holistic assessment of person and home/support system
      ~ medication management
      ~ functional assessment - ability to manage personal care and household tasks
      ~ health education to manage chronic illness
      ~ linking people with community resources
      ~ helping individuals/families to cope with anxiety, depression and dementia at home
      ~ planning for future health care decisions such as alternative living and advance directives

Homemaker services

Fee for Services
Individual payment is made on a private pay basis. Funds may be available for specific service fees from county agencies when financial and personal eligibility requirements are met. The number of services or visits is coordinated with each person/family to meet individual needs. Clients utilizing this service have acess to an on-call Registered Nurse 24 hours per day.
