Meeting Your Needs
Researching continued care options can be time consuming and difficult to understand. Although many services are available, it can be a maze for people trying to figure out the different levels of care, the price for the service, Medicare and Medicaid eligibility, as well as what continued care services will best meet individual needs. Our Community Resource Coordinator works with individuals and their families to identify what continued care services best suit the client’s needs. As a member of Beaver Dam Community Hospital’s Social Service Department, the Community Resource Coordinator maintains up-to-date knowledge of the resources available to persons in the Dodge County area, and will assist you to find close-to-home services for continued care needs.
